IRS/CLIENT SUPPORT
Admission Criteria:
Client support services are available to any client, or their family, in need of support in their home or community. The target population is at a higher risk for becoming disengaged from services. The client support specialist operates from a “been there before” perspective and will relate to other consumers on a different level than a professional staff member.
Format:
Client support services are delivered in the home and in the community as needed and include:
- Enhancing community integration through opportunities for community access and involvement
- Collateral contacts with significant others
- Assessing needs and planning services
- Linking individuals directly to services and supports
- Assisting the individual directly to develop, obtain, and manage needed resources
- Coordinating services/service planning with other agencies and providers
- Monitoring of the services delivery system
- On-going assessment of needs, goals, and preferences
- Advocating for the client in response to needs
- Planning for transition between services or life stages
- Coordination of transportation services
- Assistance with budgeting, shopping, housekeeping skills and personal hygiene
Discharge Criteria:
Discharge from client support services takes place when the client and/or the therapist determine that the client is able to identify their needs and access the appropriate services to meet those needs.
