Solutions for Life determines its fees for client services in accordance with a Uniform Fee Schedule which is based on the client's GROSS ANNUAL HOUSEHOLD INCOME and FAMILY SIZE.  Income includes all types and sources of income, whether taxable or nontaxable, earned or unearned, for ALL PERSONS living within the client's household or place of residence. (Exceptions may be made under justifiable or unique circumstances). 

The Uniform Fee Schedule is established and approved by the Board of Directors of Solutions for Life and shall be reviewed at least annually for reasonableness. 

The established fee for individual therapy hour is currently set at $95.00 per hour for therapy and $150.00 per hour for psychiatrist fees.  The client's responsible portion of that hourly fee is determined on a discounted sliding fee scale in accordance with the client's gross household annual income.  Solutions for Life will deduct $1,000 from the annual household income for each additional person who is dependent on that annual income.  A minimum fee of $10.00 per hour and a maximum fee of $95.00 per hour will be charged for therapy or $150.00 per hour for psychiatrist

A client is defined by Wyoming Statute 35-1-613 (a) (ix) as “any individual receiving services from a human service program authorized under this act.”  Solutions for Life shall not apply discounted sliding fee scale rates to agencies, organizations and third party payers.  In accordance with the Mental Health Contract, Solutions for Life shall apply the discounted sliding fee scale only to individual persons who directly receive mental health services as enrolled clients.  Correspondingly, under the Substance Abuse Contract, Solutions for Life shall apply the discounted sliding fee scale only to individual persons who directly receive substance abuse services as enrolled clients. 

Solutions for Life shall actively assist clients in their pursuit of any appropriate third party pay entitlements for treatment services, and shall bill all available and appropriate third party funding sources for treatment, prior to using funds made available under State Contracts. 

Insurance companies are billed at the full fee rate of $95.00 per hour for therapy and $150.00 per hour for psychiatrist fees.  At no time will the agency collect a combined total of client and insurance fees of more than $95.00 per hour or $150.00 per hour dependant on the service provided.  If there is overpayment by client or insurance, the overpayment will be promptly refunded to client. 

Clients must provide PROOF OF INCOME to the Agency as soon as possible upon entering services (required at the first visit) for determination of their responsible portion of fees, by supplying their most recent tax return form or W-2(s).  If these are not available or if the client's current financial situation has changed substantially from the prior year, the client may provide four consecutive current pay stubs from which an estimated annual income may be computed.  All proof of income provided to Solutions For Life will be photocopied and placed in the client's file to verify the income and discounted sliding fee scale to be applied.  If a client is unemployed and has no identifiable source of income, a statement attesting to this fact must be signed and placed with the client's chart, and the client will be charged in accordance with the minimum fee rate or $10.00 per hour. 

If the client feels he/she is unable to afford services at the designated rate, the therapist may make request for reduced fees.  The therapist should submit their written request with explanation to the Executive Director for review and approval prior to any change being made to the client’s account. 

If no proof of income is provided, the client will be charged at the maximum fee rate.  If adequate proof of income is provided within 90 days of the initial service date, prior charges will be adjusted to reflect the client's fee rate. 

If the client is an adult currently living with parent(s) or living with a non‑relative, and there is a refusal to disclose full household income, the client will be charged the full fee for the service. 

If a client is eligible for Medicaid, they must supply a copy of their Medicaid card monthly, to be photocopied and placed in the client's chart.  They must also provide proof of income annually in case Medicaid denies charges because of ineligibility. 

No person shall be denied services based solely on their inability to pay for such services.
