CASE MANAGEMENT
Admission Criteria:
Case management services are available to any client, or their family, in need of support in their home or in the community. These supports include accessing medical, psychiatric, social, educational, vocational, and/or other services essential to meeting basic needs and to promote community stabilization and integration.
Format:
Case management services are delivered in the home and in the community as needed and include:
Enhancing community integration through opportunities for community access and involvement
Collateral contacts with significant others
Assessing needs and planning services
Linking individuals directly to services and supports
Assisting the individual directly to develop, obtain, and manage needed resources
Coordinating services/service planning with other agencies and providers
Monitoring of the services delivery system
On-going assessment of needs, goals, and preferences
Advocating for the client in response to needs
Planning for transition between services or life stages
Coordination of transportation services
Assistance with budgeting, shopping, housekeeping skills and personal hygiene
Discharge Criteria:
Discharge from case management takes place when the client and/or the therapist determine that the client is able to identify their needs and access the appropriate services to meet those needs.
