PHILOSOPHY:

Solutions For Life is committed to serving the needs of the community through direct outpatient service, outreach programs, consultation, education, and community involvement.

The policies pertaining to the operation of SOLUTIONS FOR LIFE stresses the importance of communication, and fosters a positive environment in order to facilitate treatment and personal growth.

All services and programs are based on a client-centered approach and their desires, goals, preferences, and input are incorporated into all treatment/service activities.  Whenever possible, existing client supports such as family, friends, other providers, and the community are included in the process.

Through a proactive stance and with sensitivity to the changing needs and attitudes of the diverse community served, Solutions For Life will continuously refine outcome-driven services.

PROGRAM DESCRIPTION:

Any client, regardless of race, ethnicity, religion, gender, sexual orientation, age, income level, source(s) of financial support, physical handicap or disability, type of mental-health or substance-abuse disorder, or any other discriminatory factor, may be eligible for mental health services.  Solutions For Life utilizes a holistic approach incorporating multiple therapeutic interventions for the prevention and treatment of mental health disorders.

Clients either present themselves for outpatient services or are referred by a number of external/internal referral sources. They make the appointment themselves or they may be referred by a local referral source. The client or the referral source calls to arrange a mutually acceptable appointment for a clinical assessment.  They are offered an appointment time for this assessment within 72 hours and may accept that appointment time or set an appointment time that is best for them.

The clerical staff receives the call and completes a Request for Services form that lists the clients presenting problem, name, phone number, address, referral source and any pertinent insurance considerations.  Clients are then offered an appointment within the 72-hour time frame to do the initial paperwork and the clinical assessment. When clients present for their initial appointment, they will:

·         Be informed of their client rights

·         Sign client consent for treatment

·         Be assisted in arranging for billing/payment arrangements

·         Complete demographic data entry

The primary purpose of the program is to prevent mental health disorders or to obtain maximum function and quality of life for persons with mental health disorders.  The staff will work with the individual to ensure equal accessibility to a variety of services such as:

·         Case management

·         Clinical assessment (intake)

·         Community based treatment services

·         Continuing care

·         Emergency services

·         General outpatient services (individual/family therapy, and group therapy that are both agency and community based).

·         Prevention education

·         Psychiatric services

·         Psycho-educational groups

·         Referral to other services or providers, advocacy groups and self help

·         Skill Training

·         Supports and services offered by other agencies

 

Individualized treatment services will be provided as identified through the clinical assessment, psychological testing, and treatment planning.  Fees are determined on a sliding fee scale based on gross annual household income, or according to contract requirements.

Emergency services are provided for persons in need of immediate assistance by calling the SOLUTIONS FOR LIFE phone number to reach the on-call therapist.
